- Birth certificate
- Proof of residency (utility bill or mortgage/lease agreement within the last 30 days, showing account holder's name) If you do not have proof of residency, please contact the registrar.
- Immunization record or completed exemption form
- If applicable: Legal court documents (name change, custody arrangements, etc.)
