Summer Reminders for School Immunizations and Sports Physicals
Over the summer break is a great time to ensure your child’s immunization record is up-to-date. The full guide of required immunizations by the Oklahoma State Department of Health for the 2025-2026 school year can be viewed online, but a few helpful reminders are highlighted below.
Pre-Kindergarten students need the DTaP (diphtheria, tetanus, pertussis), MMR (measles, mumps, rubella), and Varicella (chickenpox) vaccines.
All students entering the 7th grade need the Tdap booster vaccine before school starts.
Parents are asked to bring updated student shot records to Back-to-School Night on August 12 or you may email or drop it off at the school during summer office hours (Mon.-Thurs. 8am-3pm). Exemption forms are also available upon request.
While scheduling doctor visits this summer, don’t forget that all incoming 6th-12th grade student-athletes and band members need an annual sports physical. Once the physical exam is completed, please upload the OSSAA physicals form to your student’s ArbiterSports/FamilyID account online, along with other medical information needed for sports participation. The student account process instructions and the updated OSSAA physicals form are available on the school website under the “Athletics” tab or visit www.perry.k12.ok.us/apps/pages/athletics.
The Noble County Health Department is offering sports physicals on July 8 and July 22 at the Health Department, located at 300 East Fir, Perry. By appointment only - please call to schedule 580-336-2257.
The first day of school is Thursday, August 14 for all Perry Public Schools students.
