Online Free/Reduced Meal Applications are safe and secure. Apply online in the comfort and privacy of your own home. The online service is available 24/7 anytime, anywhere there is an Internet connection! Your data is transmitted to the Child Nutrition Department the same day you apply, allowing for quicker processing so you can receive benefits faster.

We strongly encourage parents/guardians to have applications completed by September 15 to ensure they are approved by October 1.

Free/Reduced Meal Applications may also be picked up at the Child Nutrition Office at 901 Elm St., or any school office.

Why complete a Free/Reduced Meal Application?

It may be the most important form you complete for your school!

Every child that qualifies for free or reduced benefits:

Helps to keep the cost of meals lower.
Helps to determine the funds the district may receive from both State and Federal Programs.
Helps to benefit all school programs.

Students who qualify for free or reduced-price meal benefits may also be eligible for discounts on testing fees (such as ACT). You will need to supply your approval benefit letter to the school counselor that is sent to parents after their application has been processed by the child nutrition department.

More Information

Only (1) one form per family is needed, listing all students and family members who may reside in the household.
Only applications for the current school year will be processed.
Parents and students are responsible for the cost of meals until the family’s free/reduced application is processed.
Applications will be processed within 10 business days after receiving the application in the Child Nutrition office.
All applicants will be notified by mail of the result of the application.

FREE & REDUCED MEAL PROGRAM APPLICATION

It is important to remember that free/reduced meal applications must be filled out every year for a continuation of benefits to be determined. Completing the free and reduced meal application is incredibly important for our district. Students may qualify for free and reduced rates for breakfast and lunch based on household size and income. Each application helps the district with state aid funding, which ties directly into every district program and department. Many special services are made possible as a result of the district’s free and reduced percentage.

Please take a moment to fill out an application on the student records portal button in the parent portal. If you need computer access to fill out the application on the parent portal or need assistance, please contact Emily Miner to setup an appointment to complete the application. The application is also available by paper in the Registrar/Child Nutrition office located at 901 Elm St. and your child’s school office, or an online form can be found on our school website.

Every completed application contributes to helping our school qualify for additional funding from the federal government to help support teaching and learning. Those funds directly impact every district program and department, including instruction, athletics, technology, etc. Students who qualify are also eligible for many other benefits, such as:

NOTE: If you are already on SNAP or Medicaid, you can automatically qualify by entering your case number. Please complete the top portion of the application with student name, grade, birthdate, case number, and parent/guardian signature - and you’re all done! (If you are interested in applying for SNAP, we can help with that too!)

Free & Reduced Lunch Eligibility Scales

Perry Public Schools today announced its policy for free or reduced-price meals for children served under the National School Lunch Program (NSLP) and the School Breakfast Program (SBP). Local SFA officials have adopted the following household size and income criteria for
determining eligibility:

Children from households whose income is at or below the levels shown are eligible for free or reduced-price meals. To apply for free or reduced-price meals, households should fill out the application online. They may access the application at www.perry.k12.ok.us or by visiting the Child Nutrition Office at 901 Elm. The information provided on the application is confidential and will be used for the purpose of determining eligibility and may be verified at any time during the school year by school or other program officials. Applications may be submitted at any time during the year.

For SFA officials to determine eligibility, households receiving Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance to Needy Families (TANF), or Food Distribution Program on Indian Reservations (FDPIR) benefits must list the children·s names, a SNAP, TANF, or FDPIR case number; and the signature of an adult household member. All children in households with any household member receiving benefits under assistance programs are eligible for free meals. Children in Other Source Categorically Eligible (i.e., children categorized as homeless, migrant, runaway, Head Start, Even Start, and foster children, if known) are also eligible for free meals. IF YOU DO NOT LIST A SNAP, TANF, OR FDPIR CASE NUMBER, then the application must have the children's names, total household members, the amount of income each member received last month, the signature of an adult household member, and the last four digits of the adult's social security number or the mark the box if he or she does not have one.

Foster children also categorically qualify for free meals/milk, regardless of child's income. If you have foster children living with you and you wish to apply for such meals or milk for them, please complete the application as instructed.

Under the provision of the policy, Child Nutrition Director Emily Miner will review applications and determine eligibility.

Parents or guardians wishing to make a formal appeal may make a request, either orally or in writing, for a hearing to appeal the decision to Superintendent, Chad Wilson at 900 Fir Ave. Perry, OK 73077.

The school's policy statement contains an outline of the hearing procedure.

DIRECT CERTIFICATION: Any member of a household currently certified to receive SNAP, TANF, or FDPIR benefits will be notified of the enrolled children's eligibility and that the enrolled children will be provided free benefits unless the household notifies the SFA that it chooses to decline benefits. If SNAP, TANF, and FDPIR households were not notified of their eligibility within a few days of 08/21/2025, they will need to submit an application or contact the district with any questions or concerns.

To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant's name, address,
telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
